Monitoring(RPM)
Remote Patient Monitoring (RPM) of chronic pain patients and post-acute orthopedic surgery patients involves Outcomeus collaborating with healthcare providers to monitor and measure patients outside the traditional care setting, using digital medical devices, clinically-validated assessments of function, and other key measures. The collected data is then analyzed and reported to providers for care management. Automated feedback, workflows, and predictive analytics are built into the data collection, while collaborative communications help drive better outcomes as patients are assessed and care is delivered remotely.
Remote Therapeutic
or Treatment
Monitoring(RTM)
Remote Therapeutic or Treatment Monitoring (RTM) services monitor health conditions, including musculoskeletal status, therapy (medication) adherence, and therapy (medication) response. RTM is intended for the management of patients utilizing medical devices that collect non-physiological data, and the data can be self-reported as well as digitally uploaded.
Chronic Care
Management(CCM)
Outcomeus' Chronic Care Management (CCM) for chronic pain and post-acute orthopedic surgery patients involves patients with additional chronic conditions, such as heart disease or diabetes. Outcomeus develops a comprehensive care plan with the provider which lists health problems and goals, additional providers, medications, community services needed, and further information about patient health. CCM also explains the care needed, and how providers will coordinate. To ensure more positive outcomes, Outcomeus will work with the patient and their providers to educate patients, manage symptoms, coordinate care providers, and ensure treatment compliance.
Chronic Pain
Management(CPM)
Outcomeus' Chronic Pain Management (CPM) involves the monthly assessment of diagnosis; assessment and monitoring; the administration of a validated pain-rating scale/tool; the development, implementation, revision, and/or maintenance of a person-centered care plan that includes strengths, goals, clinical needs, and desired outcomes; overall treatment management; facilitation and coordination of any necessary behavioral health treatment; medication management; pain and health literacy counseling; any necessary chronic pain related crisis care; and ongoing communication and care coordination between relevant practitioners furnishing care, e.g. physical therapy and occupational therapy, complementary and integrative approaches, and community-based care, as appropriate.
